I would be as certain that it has happened as JP is certain that there are no forms of intelligent life in the universe. D man passes puck to other d-man and heads off the ice at the end of his shift, other dman passes to his new partner who scores.

Could also be if a D-man was just out of the penalty box and had an immediate breakaway, odds are the PK unit already on the ice would have 2 other d-men.
